Accidents and Incidents

At William & 玛丽, we investigate work-related accidents and incidents involving personal injury, illness and significant property or environmental damage. Each investigation is an objective and thorough analysis of the incident that accurately describes the event and suggests ways to prevent the future occurrence of similar accidents. The investigation information is then used to improve our environmental, health and safety processes.

The success of an investigation process is largely dependent upon how timely an accident or incident is reported. The longer the delay in reporting an incident, the harder it is to collect accurate information, identify root causes and determine effective solutions. All William & 玛丽 employees are expected to report an accident or incident within 24 hours of its occurrence. Some accidents and incidents have specific instructions:

Work-related personal-injury accidents or illness

Injuries or illnesses that involve medical treatment beyond first aid will be investigated. Injuries or illnesses that only involve first aid may be investigated if the supervisor or EH&S Office believe it would provide beneficial learning outcomes.

To report a workplace accident or injury, the employee and supervisor must complete all required documents on the University Human Resources Workers' Compensation webpage and submit them to Human Resources as directed.  

Spills

Spills of oil, chemicals or other hazardous materials of any amount into a sink, floor drain, exterior storm sewer or site drainage ditches will be investigated. Spills involving petroleum products will be investigated. Report spills to the EH&S Office.

Equipment Damage Incidents

Incidents involving equipment damage should be reported to the William & 玛丽 Police Department by calling (757) 221-4596.

Vehicle Accidents

All vehicle accidents involving state-owned or leased motor vehicles will be investigated. Reporting information can be found on the Office of Public Safety's Auto Accidents webpage.

Student Incidents/Accidents

All student employees must complete all required documents on the University Human Resources Workers' Compensation webpage and submit them to Human Resources as directed. Accidents involving students that are not university employees should be documented and reported to the department and the EH&S Office by completing the Student Accident Form. Medical care may be provided at the Student Health Center or a local medical provider for students that are not employed by the University.

Animal Bites During Research

According to the Institutional Animal Care and Use Committee, animal bites are to be immediately reported to the IACUC, the EH&S Office and the appropriate supervisor. Animal bites that occur during research should be reported by completing the Animal Bite Form.

Visitors and Guests

All visitor and guest accidents should be reported to the William & 玛丽 Police Department by calling (757) 221-4596.
